Services I provide as a Professional Health Advocate:

  • Help you prepare for your medical appointments, attend them with you (remotely as needed), and help you review any next steps.

  • Translate medical information or complicated language for you so you can make informed decisions yourself.  

  • Prepare relevant questions to ask at appointments, to boost your confidence.

  • Speak with healthcare provider(s) and others on your loved ones’ behalf (with written permission), to help you maintain updated and accurate medical information.

  • Review prescriptions, over-the-counter medications and other supplements.

  • Organize your medications into an easy-to-read table that includes any important information and cross-check them for safety, to make sure any mixture of medications are not causing you new health issues or side effects.

  • Organize and create a more accurate, in-depth medical history to provide to your healthcare team.

  • Search for extra educational material about your medical condition and its treatment options from reputable resources.  

  • Explain complex words using plain language, so you can make your own healthcare decisions.  

  • If you decide you need a second opinion, I can help you in that process.

  • Ensure you and/or your loved one understands all hospital discharge instructions.

  • Provide resources for equipment or services needed after discharge.

  • Coordinate follow up care.

  • Provide checklists to assist in the safety of your hospital stay.

  • Provide recommendations to safety proof home for successful transfer to home
